Diabetes LifeCare in the Glastonbury Western Blvd. Healthcare Center

Diabetes LifeCare is Hartford Hospital's team approach toward diabetes wellness & complication prevention. The goal of the program is to give diabetics the chance to take better care of themselves and to prevent long-term complications.

From Route 2 East

• Exit 8 (Hebron Avenue) then turn left at the end of the exit
• Left at the 4th traffic light onto Eastern Blvd.
• Take the 2nd left onto Western Blvd. and look for Building 300

From Route 2 West

• Exit 8 (Hebron Avenue) then turn right at the end of the exit ramp to Hebron Avenue
