Coordinators play a pivotal role in all phases of the transplant process. They manage referrals, evaluations, waitlisted patients, and coordinate management of post-transplant patients.
Social Worker
Transplant social workers work closely with patients and families to provide support and cope with the stress involved with transplantation. They can identify community resources for patients and their families to assist with their recovery.
Registered Dietitian
Transplant dietitians review your current nutritional status and provide recommendations to optimize your nutrition throughout your transplant experience.
Financial Coordinators
Transplant financial coordinators focus on insurance issues related to the transplant surgery and medical follow up. They can help with insurance requirements, coordination of benefits, & verify coverage pre & post-transplant.
Psychologist/Psychiatrist
Performs a thorough assessment of your mental health, your wellbeing, and your social support.
Pharmacist
Transplant pharmacists have expertise in patient education, monitoring of drug adverse interactions, and improving patient medication adherence. They will also coordinate medications upon discharge.
