Services

Inpatient

Our clinical teams care for the majority of patients admitted to Hartford Hospital. We care for medicine patients at the floor, ICU, and step down level of care. Our teams consist of highly trained, board certified physicians, advanced practice providers, and medical residents. Our goal is to provide the highest quality efficient, safe, thoughtful, and empathetic care to all who need it. Our primary teams serve as the lynchpin around which your care in the hospital revolves; ensuring the appropriate diagnostic plan, specialists, services, and followups are arranged for you during your stay.

Addiction Services

Alcohol use disorder and opioid use disorder are on the rise nationally. The most recent literature in this field indicates that hospitalizations are an ideal opportunity to start effective medication treatment for addiction and connect patients to ongoing outpatient services. Our inpatient addiction service, staffed by experienced and board certified addiction medicine providers, aims to provide both social and medical resources to all patients with these diagnoses (whether admitted for them or they coexist as a comorbid condition) in a standardized, evidence based manner. We also aim to ensure our patients have appropriate followup upon leaving the hospital, so they are able to continue on their healing journey.

Proactive Rounding Services

Our service, which is led by experienced intensive care advanced practice providers, aims to identify patients at risk for decompensation on our medical floors and pull together nursing staff, primary teams, and utilize their extensive critical care experience to implement interventions designed to act early and hopefully, avoid need for higher level of care (ie, ICU). When higher level of care is needed, our team works with patients/families to ensure patient goals of care are in line with this. They also work with bed management and the primary team to ensure the correct unit is assigned to the patient, ensuring seamless transition of care.

Transitional care services

Many patients admitted to the hospital do not have a primary care doctor or a way to obtain access to one post discharge. Our transitional care services help this population get seen in our primary care clinic soon after discharge to ensure their clinical status continues to improve. Our team also begins implementation of the post discharge care plan (ie, tests, labs, etc) and will help the patient set up primary care at the location of their choice.

Primary Care Services

We build lasting relationships with our patients and enjoy caring for them throughout the many stages of their lives. We provide yearly physicals and regular check-ups, flu shots and other immunizations, and nutritional counseling.

Taking care of a patient’s mental health is just as important as their physical health. At our Clinic, patients also have access to social support and can meet with social workers to discuss any behavioral health needs.

Providers also help patients with ongoing management of any chronic health conditions like diabetes, high blood pressure or high cholesterol.

At Hartford Hospital Community Health, patients have a whole team of professionals behind them.

The Community Care Center

The Community Care Center, part of Hartford Hospital’s Community Health, opened in 1992 as part of the Brownstone Clinics. It provides a wide range of services for patients with suspected or established infectious diseases as well as primary care services.

These services include travel medicine; evaluation and treatment of all infectious diseases including: tick-borne illnesses, viral hepatitis, sexually transmitted diseases, fever of unknown origin, tuberculosis, and evaluation of immunocompromised patients.

It is also a (Ryan White) Center of Excellence, with one of the largest HIV facilities in the state, serving approximately 500 patients. We provide comprehensive care for persons living with HIV including substance use disorder management, psychiatry, and have access the Hartford Healthcare’s referral network for any subspecialty needs.

Our Center for Gender Health creates a welcoming, supportive environment for gender diverse individuals to receive comprehensive healthcare and has access to experts in endocrinology, gynecology, plastic surgery, behavioral health and primary care.
