Diabetes LifeCare at the West Hartford Wellness Center in Blue Back Square

Diabetes LifeCare is Hartford Hospital's team approach toward diabetes wellness & complication prevention. The goal of the program is to give diabetics the chance to take better care of themselves and to prevent long-term complications.

From 84 East or West

• Exit 43 (West Hartford Center - Park Road) and merge into the far left lane.
• At the end of the exit ramp, take a left onto Park Road.
• Take the first right at the light onto Raymond Road.
• Go to the second traffic light and take a left onto Memorial Road.
• Take the first left onto Isham Road and proceed into the Memorial Parking Garage.
• Take the elevator to the 4th floor and look for Room 425
• There is fee for parking
